Athletic Clearance

ATHLETIC CLEARANCE PROCESS

The Athletic Clearance process is online!  The instructions in the pdf below will help guide you through the process.  

All athletes must complete the Athletic Clearance Process to participate for each sport. 

 


Please refer to your school for specific instructions, timelines and requirements.

All incoming and returning student-athletes are required to have an athletic physical completed as part of the clearance process. If you did not attend Clearance Day, please click here to download, print, and fully complete the physical form with your doctor. Please complete and submit the new online clearance forms via www.athleticclearance.com and turn in your completed physical and confirmation page to your athletic department.

Note: Multiple Sport Athletes- At the start of every new sport season, you will need to sign up for that particular sport. 

 
IMPORTANT: Please keep emergency contact information and health history current at ALL times on your son/daughter's athletic clearance account. When changes are made please notify the Athletic Department and your Coach immediately.

The forms listed below are included in the new online clearance program
 
  1. CIF Code of Ethics Form – This required form will be acknowledged by students and parents via the new online program.
  2. Hold Harmless – This required form states the inherent and obvious risk to participating in High School sports/practice/games and travel related activities and gives parental/guardian consent to treat in case of emergency. The form will be acknowledged by students and parents via the new online program. 
  3. Insurance Card – CIF requires a participant's proof of insurance and carrier. This information will be inputted into the new online program.
  4. Concussion Form– States the inherent risk of possible concussions in sport participation.  It lists some signs and symptoms of athletes with concussions. The form will be acknowledged by students and parents via the new online program. 
  5. Emergency Form- This information will be completed through the new online program, which provides parent/guardian and additional emergency contacts as well as insurance information.
  6. Social Media Guidelines – The form will be acknowledged by students and parents via the new online program. 
  7. Skin Infections and MRSA Information - The form will be acknowledged by students and parents via the new online program. 
  8. CIF MRSA Information - The form will be acknowledged by students and parents via the new online program. 
  9. Sudden Cardiac Arrest Information - The form will be acknowledged by students and parents via the new online program. 
  10. Concussion Parent Fact Sheet - The form will be acknowledged by students and parents via the new online program. 
  11. Concussion Athlete Fact Sheet- The form will be acknowledged by students and parents via the new online program. 
  12. NFHS Parent's Guide to Concussion- The form will be acknowledged by students and parents via the new online program. 
  13. CIF Concussion RTP/RTL Protocol- The form will be acknowledged by students and parents via the new online program. 

Frequently Asked Questions

What forms will my son/daughter need to return to the athletic office?

A confirmation email page will require a physical signature from both parents and student-athletes. This page, along with your physical, will be submitted to the athletic department for review.
